In India, the employers are required to provide one first-aid box for every 150 workers. Each first-aid box has to be kept in charge of a responsible person who holds a certificate in first-aid treatment recognized by the State Govt. and who should be available during the working hours of the workplace. Usually the only first-aid certificate which is accepted is that which is issued by the St. John's Ambulance. The basic certification training curriculum involves 3 to 8 days while the recertification usually involves one day training. Given that, it is a legal requirement to provide first-aid at the workplace, the biggest customer for the first-aid services are all the organizations employing people. One of the important components of the first-aid is Cardio-Pulmonary Resuscitation which has undergone significant changes over the years due to increasing research evidence on survival. Some years ago the recommendation was 15 :2(chest compression: breaths) which got changed to 30 2 (chest compressions: breaths) while the latest guidance recommends 100 chest
compressions per minute by lay persons while the chest compressions and breaths is limited to be provided only by the trained first-aiders.

There is large inter-state variation in India in the patterns and trends in underweight. In six states, at least one in two children are underweight, namely Maharashtra, Orissa, Bihar, Madhya Pradesh, Uttar Pradesh and Rajasthan. The four latter states account for more than 43 percent of all underweight children in India. National Family Heath Survey (NFH5-III) revealed that:

• 38.4% of children under age three are stunted, that is too short for their age and 46% are underweight that is too thin for their age.

• Wasting, defined as an abnormally low weight for the child's height affects 19% of children under age three with a slight deterioration from 1998-99.

• Overall, girls and boys are about equally likely to be undernourished. Under­ nutrition is higher in rural areas and is strongly correlated with the level of maternal education showing a two-fold difference between non-educated mothers and 10 year and above educated mothers.

• Most children under age three are anemic (79.2%).

• Malnutrition has been estimated to be associated with about half of all child deaths and more than half of child deaths from major diseases, such as malaria (57 percent), diarrhoea (61 percent) and pneumonia (52 percent), as well as 45 percent of deaths from measles (45 percent). In India, child malnutrition is responsible for 22 percent of the country's burden of disease.
Without having the detailed understanding of the actual causes/etiology of such widely prevalent condition, neither at the individual level nor at the community level problems can be viewed, discussed and tackled ultimately.

5. Write a conversation in about 20 sentences between yourself as nurse and a patient who is being discharged. Discuss the precautions that need to be taken regarding health and hygiene after surgery.
